We are seeking a skilled and empathetic Physical Therapist to join our team. In this role, you will be responsible for providing individualized rehabilitative care to our patients in a home health setting. Your expertise will help patients regain independence, reduce pain, and improve mobility, ultimately enhancing their quality of life.
Key Responsibilities
• Patient Assessments: Conduct thorough evaluations of patients’ physical abilities, medical history, and limitations to create personalized treatment plans.
• Treatment Plans: Develop individualized rehab programs based on assessments and patients’ goals, including therapeutic exercises, manual techniques, gait training, and modalities.
• Patient Care: Provide hands-on interventions to enhance mobility, strength, and flexibility, while monitoring progress and adjusting treatment as necessary.
• Family & Caregiver Support: Educate patients and their families on exercises, techniques, and injury prevention strategies to continue rehabilitation outside of sessions.
• Documentation: Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and outcomes.
• Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care and recovery.
• Discharge & Follow-Up: Discharge patients when goals are met, providing recommendations for continued care or referrals as needed.
• Other Duties: Perform other related tasks as assigned, ensuring flexibility in patient care as necessary.
What We’re Looking For
• Qualifications: Licensed Physical Therapist with a valid state license.
• Experience: Previous experience in a clinical or home health setting, especially with geriatric care, is preferred.
• Skills: Strong assessment, communication, and organizational skills.
• Patient-Centered Care: Empathy and the ability to work with patients facing various physical challenges.
• Independence: Ability to work independently with a flexible schedule.
• Tech Proficiency: Experience with electronic medical records (EMR) and Microsoft Office Suite.
• Other: Must pass a criminal background check and drug screen.
